Title: Health Program Manager
Responsibilities:
Disabled Elderly Day Care facility seeks Health Program Manager to collaborate with other health professionals to evaluate disabled elderly clients’ medical or physical condition to assess and manage client needs.
Manage and refer disabled elderly clients to community resources to assist in recovery from mental or physical illness and to provide access to services.
Monitor, evaluate, and record disabled elderly client progress according to measurable goals described in treatment and care plan.
Develop and modify treatment plans to comply with changes in elderly disabled clients’ status in consultation with medical professionals.
Utilize consultation data and social work experience to plan and coordinate elderly disabled client care and rehabilitation, following through to ensure service efficacy.
Supervise and direct other health workers providing services to disabled elderly clients.
Requirements:
- Master’s Degree in any field
- 1 year of experience in Elderly Care as VP, Manager, or other.
- Experience collaborating with other health professionals to evaluate disabled elderly clients’ medical or physical condition to assess and manage client needs;
- Experience managing or referring disabled elderly clients to community resources to assist in recovery from mental or physical illness and to provide access to services;
- Experience monitoring, evaluating, and recording disabled elderly client progress according to measurable goals described in treatment and care plan;
- Experience developing and modifying treatment plans to comply with changes in elderly disabled clients’ status in consultation with medical professionals;
- Experience utilizing consultation data and social work experience to plan and coordinate elderly disabled client care and rehabilitation, following through to ensure service efficacy; and
- Experience supervising and directing other health workers providing services to disabled elderly clients.
